I would also certainly say that it was trump's primary fault - as with everything else in his administration where the buck should stop, but never seems to. However, she definitely didn't help herself with both her lack of preparation for, and antagonism toward, Congressional meetings. This was partly to please trump, of course, but she took it to a whole other level and showed no sense of proportion.
