While I applaud RFKjr for leaving his heroin habit behind, his brain is undeniably altered from the years of addiction. He is still a junkie.

Indeed. I recall reading (in some place I'm too lazy to find again) that the brain worm is something his heroin damaged brain grabbed on to, possibly as a coping mechanism to keep from taking full responsibility for the heroin use. He found a doctor that suggested a parasitic worm as one remote possibility for his brain damage and ran with that.

While the brain worm makes for a lot of fun by his critics, it's unlikely to be true. It's far more likely to be the long term result of heroin use.
