The reason the Dukakis photo op was so deadly to his campaign is that they were able to tie it into the attack that the GOP often runs against Democrats - that they're weak on defense. The photo of Dukakis looking goofy and dwarfed by the tank and helmet hung that story on Dukakis in a powerful way, on an issue that people cared a lot about.
But Donald Trump making an off-color reference to another person's body? It's hard to see what that does. It's nothin we don't know about Trump already (the video of him making fun of a reporter's disability is eight years old already). And it doesn't reinforce anything that Democrats really have to say about Republicans on an issue that people care about.
