The COLA increase is based on the overall inflation of goods and services across the entire economy not just if medical services.

There is a “hold harmless” provision (google it for a better explanation) that limits your Medicare Part B increase to the Sec Sec benefit increase. But that provision only applies to the base Part B premium, not to Part D or to IRMMA additions.
My suspicion is that you are subject to IRMMA, so that your total Medicare premiums went up more than your Sec Sec benefit.
You didn’t mention any federal income tax withholding. That may also impact your net check.
