My biggest fear in this Iran mess is that Trump, who is pathologically incapable of acknowledging error or admitting defeat, is backed into a corner over the Straight of Hormuz and his inability to unilaterally end this war and reaches the conclusion that only escalation is possible. He’s already hinted at the use of massive force, so nukes are clearly on his mind.
If he is truly mad, and I believe he is, who will be the adult in the room and stop any suggestion of a nuclear option?

He doesn't have that kind of patience. And he's all about public perception. There is no way to "win" this without using nukes. Just like 'Nam. Smash their military, and they still have the asymmetric. And they can continue to disrupt Hormuz. That looks bad. He either goes home with his tail between his legs, or he nukes them. Plus, I think in his mind he would look "tough".
I just hope someone can stop that process. Congressional approval isn't needed to launch nukes. Someone in Congress needs to stop this action NOW. Before it gets to that point. Impeachment would be the obvious route, but I seriously doubt that would happen. Some committee needs to say "no" to him, and have the ability to enforce it. I don't know if that's possible, structurally (nevermind politically).
